You're invited to the 3rd annual Team Conference for Hospice and Palliative Care. This registration is for virtual access to the conference.

The Team Conference will be hosted by:

• HPNA
• Association of Professional Chaplains (APC)
• Physician Associates in Hospice and Palliative Medicine (PAHPM)
• Social Work Hospice + Palliative Care Network (SWHPN)
• Society of Pain and Palliative Care Pharmacists (SPPCP)
• Transforming Chaplaincy

Conference tracks will include grief and bereavement, team dynamics and symptom management.

Refund Policy:

Registration fees are refundable, minus a $75 administrative fee, if the cancellation request is received in writing to [email protected] by September 13, 2024. Cancellations received in writing between September 13-20 will be refunded minus a $150 administrative fee. Please allow a minimum of 10-14 days for refunds to be processed. After September 20, 2024, no refunds will be given. No memberships will be refunded.
